The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
Patient Forms
New patient forms are available electronically. Once you have a scheduled appointment the forms will automatically be sent to you via text. If you do not have a smart phone, we will provide you an iPad to fill out your forms when you arrive at our office. Filling out your forms a head of time will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please call our office if you have any questions.

Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

* PPO: (Preferred Provider Organization) is the most common form of insurance. They provide members with a list of participating dentists to choose from. The dentists on this list have agreed to a lower fee schedule, which provides you with greater cost savings.
Cancellation
If you are unable to keep an appointment, please give a 24 hours notice so we can offer this appointment to another patient. If you happen to have missed 2 consecutive appointments without notice, you will be dismissed from the practice.
